New US Passport Requirements Effective February 1, 2004
For individuals who

- have never had a passport or
- lost a valid passport or
- whose passport was issued more than 15 years ago or
- whose most recent passport is damaged or mutilated or
- whose most recent US passport was issued prior to 16th birthday.

1. Certified birth certificate or naturalization certificate or passport issued over 15 years ago.

2. Passport photos (2X2 with white background). Photos taken at our location.

3. Completed Passport Application Form DSP-11 (for those who have lost a valid passport, you must also complete Lost Passport Form DSP-64). Mailing Address Must Be Left Blank On Passport Forms.

4. Standard Government Fees plus reasonable service fees apply $145.00 for adults and $130.00 for children under the age of 16.

5. A letter of authorization.

6. Proof of departure date- computer generated flight itinerary from the airline or a travel agent.

7. Completed Service Order Form.

8. Fees and Payment (Service fees depend on type of service requested).
